You can add a Consent form when you:
To add a Consent form to the Client's card, do the following:
- Go to the HCP's section and select the Client from the list.
- Click the Edit button.
Note: If you need to add a consent form when adding a new client, you can skip steps 1 and 2.
3. Go to the Contact Info section and locate the Consent Form field.
4. You can:
- Open the consent form and add a client's signature
- Take a picture of the consent form and add it to the Client's card.
- Select the consent form from the gallery.
Open the consent form and add a client's signature
To open the consent form and add a Client's signature:
- Click on the icon
in the Consent to the Collecting and Processing of Personal Data.
2. Scroll down the consent form and click on the signature place.
3. Ask the Client to provide their signature.
4. Click the Sign button.
Note:
- Click the Clear button to clear the client's signature. Then you can ask client to provide their signature again.
- Click the Cancel button to close the Signature dialog without saving the signature.
Make a photo of the consent form
To make a photo of the consent form:
- Click on the icon in the OCM consent section and select the Photo of the consent form.
- Make a picture of the document, and click the icon to attach the photo.
Note: To make a new photo, please click the retry icon.
Select photo from the gallery
To select the photo of the consent form from the gallery:
- Click on the icon in the OCM consent section and select "Select from gallery" option.
- Choose the photo you need. The system adds the selected picture.
